Troop forms and files

  ABC Medical Forms (Parts A, B1, B2, C)

  Parts A and B are needed for most activities. Part C is required for 72+ hour events.

  Allows the troop to share photos on troop channels without using a Scout’s full name.

Quick rule: If your event lasts 72 hours or more (summer camp, jamboree, Wood Badge, multi-day trek), you need Part C signed by a licensed medical provider.
